DescriptionRecords consist of the completed form of complaint or report of the incident involving sexual violence and/or sexual misconduct, all material collected or created by the Intake Assessment Team related to the complaint or report, the referral document shared with the Receiving Offices (if applicable), VP report for systemic discrimination cases (if applicable), relevant documentation submitted by the reporter/complainant, and an investigation report (if applicable), summary of early resolution, and summary of findings and outcomes from a formal complaint created by the receiving offices (if applicable).
