DescriptionRecords consist of information used to determine whether a student-athlete has received medical clearance to compete in Varsity sports, including but not limited to pre-university medical screening questionnaire, doctors' notes, Varsity Medical Profile Forms, Doctors' Examination Forms, hardcopy print of Electrocardiograms, medical test records, related documentation and correspondence.
Retention TriggerLast activity recorded on file, or the day on which the patient reached or would have reached 18 years of age, whichever is later.
